HRA (Health Reimbursement Account) plan - considered by most to be our best plan and usually results in the lowest out of pocket costs per year. The numbers below are for a family (employee+partner+unlimited kids)
Company contributes $3000/year to your HRA. Unused funds roll over year to year with no cap. $4500 deductible but really it’s only $1500 (unless you have rollover funds then it’s even less) considering the first $3000 is paid out by the HRA. 10% co-insurance after that up to your $6800/$7500 (individual/family) out of pocket max for the year. All preventative care is covered at 100% and not subject to the deductible. Premiums for a family for this plan are $176 per pay check ($352/month).
Vision is through VSP and is $8/check for employee+spouse (I think it’s ~$16/check for the whole fam). Includes yearly exam, lenses, frames and contacts with reasonable allowances and copays.
Dental is included with any of our health plans. Can’t remember the details off hand but it’s pretty standard as far as I can tell (free cleanings twice yearly and good discounts/allowances on dental work).
I have put the HRA plan to the test over the years (surgeries, pregnancies, etc) and it’s great.
