I believe it's required if you disclosed any training/checking failures in your application or virtual interview. Otherwise it is not used. I never got the FHA and received the F2F invite after my virtual interview.
Thanks for the heads up! I completed my "video" interview recently and have been fortunate enough to not have any checkride failures in my background; I've been anxiously awaiting communication from JB and was wondering if I should be expecting the FHA. Thanks for the clarification.
