We need the option to fly. No questions asked. Passengers need to understand that they may or may not get to their destination. Based on flight crew willing to subject themselves to the risk.
The flights that are accepted then need have additional pay based on increased risks.
Management doesn’t like it? Fly the sick people yourself.
