The government. The idea the the Surgeon General and health advisors in the government want to save EVERY life is a joke. If that was the case they would argue to ban alcohol and cars. There is A number. The President and each governor gets to decide it after discussing it with their advisors (both medical and economic). They will never say what their number is though.
