Some hotels assume van drivers get tips and pay them less, but not all do. In my experience (9 years old now) less busy hotels don't necessarily have dedicated van drivers. These employees make a normal wage. Sometimes they are behind the desk, and sometimes the drive the van. In those cases, tips ARE extra.
As a pilot all that isn't worth the time thinking about. Just give them $2 and worry about something more important.
