Not necessarily.
School records should indicate the course of training the student was enrolled in, 61 or 141.
School records should also indicate who the (primary) instructor was.
You have your logbook with the students name in the remarks section.
Both records combined should suffice.
I never split the dual given column and I’ve been Chief Flight Instructor 141.
Do yourself a favor though and ( if you’re not already) develop some sort of short hand entry for your Remarks column.

The students training records should indicate exactly what was or wasn’t covered during the lesson.
