I was the Co-pilot of a relief crew crossing the pacific. After waking up, I walked into the flightdeck to figure out when exactly they wanted me in the seat. I noticed the "yokes" on both sides were turning right, yet the airplane was wings level and a bit more nose up than usual. Both pilots were playing some video game on the center console.
The flight engineer had started a fuel transfer and then decided to get out of the seat to use the restroom. Then he decided to make lunch at the galley. (two big no-nos). The result was a 17K fuel imbalance between the wings while the limit was 4K. I don't know what the max theoretical fuel imbalance is, but we had to be very close to it.
