TOE really means “Atlantic Ocean Crossing” haha. There are a few extra procedures and HF radios and CPDLC. Not rocket science, but if you haven’t done oceanic/NATS before it is necessary training.

You’d need to look at the FOM for exact hours/landings, but the footprint is basically a 4-5 day domestic OE trip then at least one over-and-back trip across the pond. Current wait time seems about a month from 44X to OE then another 2-3 months for TOE.
