The company got an article written about them in a national publication about their practices around not letting pilots get vaccinated if they have work. The FAA requires you to ground yourself for a period after getting the vaccine. The paperwork let’s you get the vaccine and notify the company so that your flying gets covered without going through the traditional sick call. It’s just a streamlined way to accomplish this.
