There's a couple of things here though - it hasn't been a problem for XX years, it's part of the Acceptance and Before Start flow. Bottom line, people are not doing their job. If they were doing their job, then it wouldn't be happening. Hence, drop the hammer. That's how it works in the theoretical world anyhow, I'm with you that we need to know why the people that forgot, forgot, what lessons we can learn from it and improvements we can make.

Just a thought - forced upgrades, lots of younger people taking the helm. What I've seen from my FOs is that they are always in a rush ! Always wanting shortcuts and flying fast. Impulsivity is one of the five dangerous attitudes. Eh, my two pennies worth...
Its been happening for years. And we aren’t the only airline with pilots who forget the parking brake or NWS switch.
