Well, that's true.
But to take ONE example from your shop, I'm sure it was before your time there. The whole 76 in ATL was pretty amazing, considering how many layers of safety and stop gaps they went through before making that mistake.
Pretty sure it's never been disclosed, but when did the SQ (or whatever you call it) LCA doing the route qual for the CA start peeing out his butthole on that one?
Point being, in this day and age, you can rationalize/defend/excuse anything, just depends.
