AQP can definitely be failed. And it happened a lot when I worked there. It was almost always during the LOFT portion. IMO, the instructors come up with scenarios that fatigue the pilots and then they screw up. Easier said than done, but during the LOFT pilots need to slow down, take their time, and do it correct.
