I mentioned the date, type of checkride, why I failed, and what I learned from it. Nothing more. Anything else they need can be obtained through PRIA.

Also- make sure you mention failures AND Incomplete lessons from ANY training event. Even those you might not have thought about that your previous carrier might mark you down as unsat. Not just your MV/LOE.
