The only place I've actually seen this is Ho-Jet. A few places like Chatauqua and PSA had their known 'hammers' back in the day.
I would suggest the Occam's Razor explanation is the quality of candidates is dropping and instead of letting them go before the type ride they are pushing them through. Eventually the APDs get tired of this game and start making a point by saying no.
