Absolutely 100%. Some screw-ups are so egregious that consequential actions are a necessity
we should demand. There’s absolutely NO reason scenarios like this should ever occur today with all the TEM, SMM, CRM, etc. that should be second nature for any 121 crew. Mistakes happen, yes, but accountability should still follow.
