True, but how many of those in comparison were caused by lack of experience, sliding through training, etc. and how many were just plain complacency or just simple human mistakes? The latter can happen to anybody, whether you have 400 hours or 40,000. The other is the direct result of hiring standards.
One is preventable, the other is a harder animal to prevent but we are doing better.
