1. At least restricted ATP. Yes.
2. If you live in the 50 states, and don't live in base, you will be assigned a gateway. They will fly you to/from your gateway, and where they want you to start you trip. If something goes wrong, they fix it. They buy you plane tickets (or pay for your mileage, or your rental car), and pay for your hotels.
If you live overseas, it is a lot more complicated.
3. QOL ranges from AWESOME, to HORRIBLE, depending on your fleet, your base, your seniority, where you live, what your family situation is, and over 1,000,000 other variables.
