Airlines can figure out how many seats per flight would be affected by fat people spilling over into the next seat, hold those seats, and then spread that cost out among the rest of the passengers through higher ticket prices.
Airlines could do that, but why would they give up revenue? If the passenger demand is high enough that they could sell single seats at those higher rates, why would they A) not already be charging that rate and B) hold back pairs of seats for a single passenger that would literally be sold at half price?
