all my employers in the last 25 years have done Pre-employment and random testing for Drugs and alcohol, so the answer is "YES", and so if yes I will just put my Employer's information.?

This is a tough one, which I would consult a professional app review service. If I remember correctly, you were previously employed overseas, where you would not be subject to the US Dept of Transportation drug screening. In that same vein, I, a career military flyer, was not subject to DOT testing, but obviously tested regularly by the military, so answered I “no.”
I would only answer “yes” if the DOT saw your results, either as an airline pilot or truck driver, etc.
