Bazinga! Losing control of a 777 because flaps 15 was set instead of flaps 5, an extreme common error on a flaps 20 take off, is absolutely unbelievable. Headsets or not you don’t dive an airplane into the ocean because you ordered a coke and got a Pepsi.
That reminds me of a story I heard from an ex-pat captain in China. it was a smaller domestic carrier, 737 I think. His first line trip, after engine started he called for flaps 5. The local Chinese FO looked at him funny, as if he didn’t understand. Captain repeats command. FO jumps up, goes to the back and returns with a cola for the captain!🥵
