I took a look at my pay stubs, and back on the first check in October, a separate line for 401K contributions was added,
with my prior YTD contributions one a now-unused line below it.

Lo and behold, my Oct-Dec contributions, on a new/separate line, total to the exact amount missing from my W2 12D* box.
It seems when accounting implemented the MBCBP, at least some of those who did not opt out had a 2nd 401k line started on their stubs, and the W2 system did NOT include that "new" line.

I did audit taxable wages vs. deductions via my final stub, and those amounts are correct, so the error seems to be limited to the 401k contribution box only.

Anyone else on the MBCBP seeing this?
Expect a corrected W-2…
