With the Jpy/USD exchange still pretty cheap. Best of luck. Normally a Flight Surgeon on base can issue up to a 2nd class medical. Course it has been a long time since I left active duty, so things probably have changed.
A military medical (DD2992 “FFD”) is equivalent to a 2nd class. At least it was 4 years ago when I retired. USAF (Kadena), USMC (Futenma), and probably USA (Torii Stariin) medical departments should be able to help you out on Okinawa.
