Disclaimer: I don’t work in pilot hiring but I did go through the process and got hired. Specifically to your situation, your single-page FHR + green folder/whatever printouts from the other systems were after the green folder are far more important. Write it out simply and professionally and it shouldn’t be a problem.

Anecdote: a friend of mine did not have their green folder and they were hired recently with a memo that basically stated “these are accurate to what the Air Force has on file, as they changed their record keeping…”
