Non contractual plans-
Appeal process is a company HR fight and if you don't agree with them, tough cookies.
DPMP and contractual HSA
Alpa can fight them on your behalf with a lot more teeth. tI'm probably about $40k ahead from ALPAs fighting, up to us going to court, and winning on my behalf.
Things DL has cheaped out on include denying an in formulary medication my wife needed from a brain tumor, trying to deny my shoulder reconstruction saying anyone but them should pay.
