As you said - just like they only show 4 events for ops day, it wasn't just one person sitting in the wrong position, and it wasn't just seat position that caused the "steep dive". It's an item that was seen frequently enough it was deemed worth spending a few minutes on during this years CQ.
Seems like the ops day lessons didn't fully sink in as we continue to have similar events occurring on the line very recently. So no doubt we will continue to receive more training.
