Strange. I've been on Advair for about nine years and nobody's ever said boo. This is the second time today I'll advise someone to call AOPA - they might be able to give you more information. And your allergy doc might have done this sort of thing for someone else already and will know what to do. I've had docs type up letters for various issues before - a form isn't always necessary. In any case it doesn't seem like there will be a big problem, or maybe I've just been lucky.
