Default High Altitude SIAP Symbology and Policies

Any military guy's willing to provide a little insight on high altitude approaches to satiate my curiosity?

The USAF Instrument Flight Procedures book and the civilian 7110.65 have given me some good info, but there are still some things I'm curious about...

There are often courses depicted using a series of heavy bold dots...I understand this depicts a high speed "penetration turn" from the en route structure..(?) is this the correct? In NOS low altitude approach plate legend, the same symbols are listed as the "lost communication" track, and most often seem to appear on STARs and DPs...

Is there any publicly available legend that depicts the different symbols?

I'm also under the impression that a privately owned civilian jet aircraft (CAT C, D, E) could request and be cleared for these procedures provided that ATC is able to accommodate the request and the aircraft is equipped to fly it...am I wrong in this assumption?
