Sorry, but can you really tell me that flight/duty times and fatigue are something that SHOULDN'T have been looked at till people were killed? I don't think in this day and age, where every bit of productivity is being squeezed out of crews it takes a crystal ball to see there would be a problem. Sadly, the FAA is generally reactive instead of proactive.

But I'm sure you've heard of ASAP before? I'm sure that's had influence on changing things. Maybe not always a reg, but I know there have been IAP's changed because of it.
