Your AME might well have you go to the FAA Med Pass and fill out all the information on line prior to going to the office.

I do not think that the AME expects you to bring some large family medical file. I did take my military medical record, but then again I knew that there was something in there that he needed to see (a military medical waiver).
To your second question - even the AME told me that he was not my *friend* and that when I wasn't answering him 'yes' or 'no' to the questions he was asking then we should caht about golf or something else NON-MEDICAL related. In other words - don't go offering up information that is not in you medical record. You are not a doctor so you shouldn't be trying to self-diagnosis too. Answer the questionnaire as honestly as you can remember and answer his questions as directly and honestly as you can remember IMO.
