Here's what I use:

- LogTen Pro
- Old World Binder
- Blank Inserts for Old World Binders
- A laser printer that can collate.

Once I have a new page, I simply print that single page to PDF, head to Kinkos, print it off, and add it to the group. Haven't had an issue, and the results look great.
