First and foremost you do not need CP approval for ML. They can NOT deny mil leave because it falls on a holiday or they are short staffed.
To answer your question though the form is on Deltanet on the CPSC page it is a fill in the blank sheet that goes to your CP.
That being said they can and will call you to verify the mil leave and they may call your commander or equivalent to see about your mil leave. For example if you take one day of Mil Leave on 25 December you can pretty much expect a call to your CC. Both times this year I have had mil leave over a holiday I received a call from one of the CPs. I did not feel threatened or harassed and I had no issues. Since then I have gone in and introduced myself to the CP so he can put a name to a face.
Unfortunately this policy is in place because there are a VERY FEW guys that may be cheating the system.
