For the Beechjet type, you won't have a Form 8, but rather a graduation/training report from UPT that the FSDO local to the base knows how to interpret. If in doubt call ahead to the FSDO and make sure they know how/are willing to help you.
This is what I was told from people at other UPT bases, that they had no problem getting it at the FSDO with the graduation report and I think an 8710 or some simliar form. I am aware of the guy at CBM charging to do this for you which is why I am asking if anyone has done it on their own, but I did find out it is very possible to do at a FSDO for free rather than a couple hundred dollars. I havea commercial multi instrument just need the type add on, I dont need to take any tests. Ive never paid for add on simliar at the FSDO, they just need the paper work for the additonal rating.
