Watched this thing on the Science Channel a while back about the design and testing of the A380. They included the engine testing with a really great shot of the intentional blowing of a fan blade at max power. The test was to ensure the shroud contained the catastrophic failure.
I'll speculate a little and say this looks like a turbine failure like the UAL 232 crash in 89. They are incredibly lucky!
