Just curious about what other airlines provide for the crew rest area.

Here at Brown, on the MD-11 we usually have a supply of plastic lined paper pillow cases and sheets, no blankets. We put the pillow cases over a few of the rattiest old fiber-filled pillows you have ever seen. I don't think the cushions in the crew rest bunk have been wiped down since the last D check.
