You need to have as much verifiable proof that you worked for them as possible. Pay stubs, W2's, everything. If you have a contact number still, call them and find out if they can give you a new fax number so that they can comply with PRIA requirements.
If you are dealing with a part time job you held for a few months duration four years ago, would a W2 be enough? This particular place has long since closed - out of business. I don't remember exact employment dates, although I'm fairly sure of the months. Would I be wasting my time applying?
