I just sent mine in and got a reply within about 2 weeks. I used the form above, got it notarized for free at my city hall and mailed it in. I know for a fact that within the past ten years, I have had one speeding ticket (in another state), a fender-bender that I was at fault on with no police report but there was an insurance claim, a seat belt violation, and one major driving infraction in which I was arrested, but that had no conviction.

I got my letter back from the NDR that states "We have searched the NDR database and found no records pertaining to you." Obviously I was happy as hell that day
