Default Airline Apps employment history questions

I am working on my airline apps, mostly interested in United, and have a question regarding past employement listings. Through my early career I worked at several, now defunct, flight schools, gliderports, and dropzones. I was always a "self employed contractor" at each. Should I list each entity separately, or should I make one entry as a "Self Employed General Aviation Contractor" and just describe the various activities in the "Duties" section?

Also, under "General Experience" where it asks if you have ever held Chief pilot, DOM, check airman or other titles, would you include chief pilot/DOM positions held at Pt 91 ops or is that strictly for 135/121 positions?

Part I: Is there anyone who can vouch for your work/services that worked with you or that supervised you? If so, list them, and contact them to make sure they know you are using their names. If not, list the businesses, and be prepared to explain that you were 1099 contractor (who apparently had the unfortunate effect of causing flight schools to go out of business! ).

Part II: This is for 135/121/141 or military experience only. It won't sit well if they find out that you tried to sneak in under Part 91 here. Otherwise, every Tom, Dick and Harry would suddenly claim this experience. They want to see that you had this experience in an environment/equipment similar to a major airline.
