Summer of 17 they changed the question to include all failures, including stage and phase checks. As to do they care about them? I’m guessing they are treated just like checkrides. Not a big deal unless an excessive amount or recent.

Not a big deal in general, yes, but don't portray that in the actual panel interview. Take responsibility and own up to your mistakes no matter what the circumstances were.

Delta has hired guys with multiple check-ride failures in any of the Part 61/141/91/135/121 training programs. How do I know? I'm one of them. Guys have also been hired with not-so perfect driving/accident/incident record. Then you'll also hear guys who have perfect squeaky clean records who don't get hired. Actually, some guys don't even get called in for an interview.

No one is perfect. How you go about answering your difficulties/challenges is what makes or breaks you during the HR panel.
