PRIA Forms
 
When filling out the PRIA forms for SWA, they ask military to fill out the forms even though they will not be sending them in. For the previous employer/airline blocks do most people just put U.S. Navy (Air Force etc) and current SQD info or leave it blank?

Also on the reference info, flying supervisor blocks if your ten year history took you back to flight training who did you put as a contact?

I filled the forms out with my current sup's/Org's contact info.

For back at training just use your Flt/CC or Sup then. I'm sure if they have questions about your past they will find a way of looking him up or getting the info they would want.

There were some Ops O's who I couldn't get in touch with but there were other Flt CC's or ADO's who I still had easy access to, so I put down their information and then let them know that they were my reference for that time period.

So I would think that you could put down a FLT CC or even one of your instructor pilots if that's all you could get contact info for.

I think you want to put down someone you STILL know if possible. You have more of a chance of them letting you know if they got called and how the conversation went.

I don't think they really care exactly what position the person held, as long as they supervised or depended on your performance and could comment on it.

This is where maintaining good contacts really helps. Ten years put me way past UPT, but I researched and was able to find old supervisors and co-workers from friends, old Xmas cards, phone book etc. Biggest help actually, was my security clearance paperwork. I highly recommend everyone keep a hard copy of their latest, and pencil in updates to contacts as necessary.

When it comes to background or employment checks, it really pays off to have current contacts and phone #'s so investigators/employers don't have to waste a lot of time on you.

Same question on "record keeper". Any suggestions for the appropriate person to put down as a record keeper? Should I just bring a print out of my flight records?

That would be SSgt Snuffy.....Do I need to brief her like I would one of my references? Will she get a phone call, letter in the mail, or will SWA not bother if I am military?

The "record keeper" line really doesn't apply to military guys. SWA knows that military flight records are pretty tight when compared to the civilian side. I wouldn't sweat the name you put down in this block. I used my current chief pilot as my current record keeper and left all the previous blocks blank.

If you're in the Navy put the name and number of your CURRENT pilot NATOPS guy for all of your military entries.
