Default NetJets, Type Ratings and the Military

I have another NetJets app question for anyone that has applied to them.

Under the section Aircraft Ratings and Certifications, what are people putting in these blocks?

Type Rating:T-1 or BE-400 or BE-400/T-1?

Date of Initial Rating: Date first checked out in the plane or day you actually got the type on your ticket, which may have been after you stopped flying that plane?

Date of Last Recurrent: We don't really do that, so maybe last sim of the year, last checkride, or CBTs (if you do those)?

Type rating: My ticket says BE-400/MU-300.

I would give them the date you got the type rating just to play it safe. For last recurrent, I'd just put N/A. They're going to understand that the military does things a bit differently than the FAA and that some things on the app may not apply.
