Hormone replacement

Default Hormone replacement

Anyone have any experience with HRT? Late 30s and starting to feel sluggish. Anyone had this done before?

Fitness works great for that, especially strength training. I started feeling it after age 40, but weights at the gym tuned me right up.

You should be doing cardio anyway, as a pilot.

Default Hormone replacement

I’ve pretty active… In the gym 4-5 times/ week. Probably could lose a little more weight, but everyone’s different. Just wondering if anyone has done it while flying. My testosterone was way below the average according to my physician.

What time of day was the test taken?

Testosterone levels are measured through blood tests. Most doctors agree that a “normal” reading falls anywhere between 300 to 1,000 nanograms per deciliter (ng/dL). About 40% of men over age 45 will have levels that come in below that range. But a low reading by itself isn’t enough to warrant alarm.

In fact, it’ll likely have a lot to do with the time of day doctors test your blood. The best time for testing is between 7 and 10 a.m. “Different hormones have different patterns of secretion,” says Ronald Swerdloff, MD, chief of endocrinology at Harbor UCLA Medical Center. “Normal testosterone ranges are based on morning samples, when the average person is at a higher level. Afternoon tests may give a false impression of low levels.”

Swerdloff says you should get multiple tests -- at least two over the course of a couple of weeks or months. You’ll want to make sure you have low T before you take any action.

More Than Just a Number

Even if your testosterone levels are below the recommended range, you still might not have to worry. Doctors say that a reading between 200 and 300 ng/dL is sort of a gray area.

Testosterone replacement is just a report on your medical. Do suggest clearing dosage and actual drug with your AME or AMAS.

I've been on it for going on a decade now. Im 43

Make sure you are not at risk for prostate cancer before taking Testosterone. Testosterone to prostate cancer is like gasoline to a fire.

Any endocrinologist worth a hoot should be running PSA as part of your ongoing labs, and do an ultrasound of the boys to make sure before starting.

Measuring testosterone levels is a tricky one. It can vary greatly depending on what time of the day you get your blood drawn (supposed to be fasted and before 9am IIRC). Also make sure you are testing for total t, free t, and SHBG to get the big picture. Most run of the mill general physicians don't look too in depth into this issue like they should, and the normal range of total test is extremely broad which adds to the confusion.

There are online clinics specializing in men's health that will send you to a local lab and review your bloodwork to address any deficiencies. For instance, you could be deficient in vitamin D and that might be the culprit of what you're feeling.

I would vote to see an endocrinologist.. My PCM tried to prescribe a crazy high dose of Testosterone and my wife bounced it off her Endocrinologist (I was active duty at the time) and he said "screw tricare, I don't want you getting messed up and that dosage will do just that" so I would strongly encourage seeing an endo.. Most of the mens clinics are Endos. Look at the doc's bios for FACE as an add on, that is usually a sign they are an endocrinologist.

A good Endo is gold. The Endocrinologist here literally saved my wife's life, because she had some weird thing that her PCM was just writing off as "you eat more than you think and don't work out as hard as you think" (Military quacks go figure)
