If you are at a 121 carrier, especially a major one, doesnt your LTD plan pay for all those expenses?

I don't work for a major, I'm a cargo guy. When I started this process I first consulted an attorney and not my union. In hindsight, I could have consulted my union and had a lot of this payed for by the company, but I wasn't sure how much trouble this would cause. Therefore, I went the lawyer route and have payed out of pocket and will continue to do so throughout this process. Hopefully this experience and others will help others in the future. What I mean is that your union can help get these things accomplished with their own team. When you don't really know the extent of your troubles (as I did because I lied on my medical all those years), I was quite frankly fearful to involve the company in any way until I had some idea as to what my options/consequence would be. I suppose I could ask my union if they would cover these expenses now, and maybe I will. For now I am still employed (not flying) and working with the FAA through the process.
