I had the surgeon fill out the Aeromedical form for a detailed eye exam 8500-7 and sent it to OK City with a letter, letting them know I had the procedure done, was stable (referred to attached letter) and was returning to flying. That was in 2004.

Thanks guys, good to know I don't have to take an extended vacation to get my eyes fixed. It sure would be nice to wake up in the morning and be able to see without glasses or contacts.
