Default Special Issuance Letter

I just got my 2nd class renewed via special issuance after a short ordeal with Ok City. I now see that the FAA is requiring you to carry the letter at all times when excercising the privledges of your certificate. When did this start? Why would someone doing a ramp check need to know my personal medical history? Shouldn't my Ok City issued medical be sufficient? What happened to HIPPA?
