I'm kinda confused. I can't find a link anywhere to the new form that has the two new questions on it. I'd really like to read up and prepare. Until I see the form, anything else is conjecture. Does anyone have a link? I've tried doing a few searches.

https://medxpress.faa.gov/ is about the only way you can see the form and u have to create an account (a one time event)

Med express website needs to be more user friendly. With the saved login info, it seems they could also save your record of medical visits. As it is now, if you need to see the Doc, you have to list that visit for the next 3 years. For those on a 6 month FAA exam schedule, you end up listing that same event 6 times. I know its the government, but it sure seems like this could be simplified for the users.

Very easy to use! I like it better than the paper version.

It is pretty easy and quick, and it really did make my AME visit much more efficient and quick.
