Not at a regional, but let me help you out:
1. Don't have the aptitude
2. Bad attitude

In the airlines, I've seen people wash out in the sim because the airline isn't going to re-do your instrument rating. You better show up instrument-proficient.

I've also seen people wash out of IOE - they get too far behind the aircraft and show no progress as the time goes on.

Remember, attitude is everything. Have a good, positive attitude, show up prepared and you'll do fine.

Instrument skills being weak
Bad Attitude
Poor study habits
Get thrown in jail for "urinating in public" or "Running from the cops because of a noise complaint"
Inability to utilize standard calls after 4 days of sim

In the sim:
Instrument skills. A sim is a sim, its just a computer. Garbage in, garbage out.
Not knowing your profiles, callouts and flows dead cold before getting in. You will not be able to learn how to fly the manuevers if you are still trying to learn what to say or which switch to flip.

If you are good at those two things and arent a total moron at CRM then the sim is a piece of cake.

In ground:
If you lied about how many checkrides you failed or why you left an old job or forgot to put something on your application it will come back to you and you will get canned. Watched it happen to more than a couple pilots.

A good attitude, paying attention in class, and doing a little bit of studying will get you through the ground. Most people dont have to cram for hours every single night.

In addition to the above: Not knowing limitations and memory items.

People wash out at my company for:

1. Failing tests in ground school.

2. Not knowing callouts or being able to fly sim. Know your callouts and do a lot of chair flying in front of the mock up.

3. Not being able to line up and adjust altitude and speed properly on a visual approach during IOE. And visuals are what we get 90% of the time and several of them aren't easy, like the River Visual, Circle-to-Land 33 in DCA, Expressway Visual at LGA, etc. So it's kind of a big deal.

4. Whining.

I have seen the worst pilots that should be let go, but because they had great attitudes and everyone liked
Them, they were given tons of re-trains and finally for through, so in my opinion attitude is everything.
