I recently did my yearly dance with the AME to renew my 1st class medical. Passed, but was told I had to get my allergy doctor to fill out an AASI (AME Assisted Special Issuance) for an asthma medication I take, more specifically Advair (I have no side effects from this in the time I've used it for my asthma).
The AME gave me a sheet about AASI's, and what I needed to provide to the FAA concerning the medication, on it, it says I need my allergy doctor to;
"submit all pertinent medical information and current status report, to include duration of symptoms, name and dosage of drugs, and side effects"
My questions is: I know what I need my allergy doc to fill out for me, but I don't know if there is any specific FAA form I need to give my doctor to be able to fill out all the required information. My AME doesn't seem think there is. I've looked online and haven't found anything either. I'm guessing I'm just supposed to tell my doctor, "hey, I need you to give me all of this information, put it all down on a piece of paper however you doctors do it. Please and thank you". It may just be that easy, maybe not, I'm not sure! In this biz, I'm just used to needing specific FAA forms to get anything done or signed off on. Have any of you had any experience with this situation, and is there anything I'm missing regarding all of this???
