After trying to talk him out of it, a jump seater who is qualified on the aircraft (Company’s reserve FO on his/her day off trying to catch a ride) shows up. If there's ice out there, I'm not going. Simple as that. That said, a crew needs to evaluate the aircraft's condition each time they take the runway, especially in freezing conditions. Ice on the wings? What type? Ice? Snow? Slush? Is it sitting on top of deicing fluid?
Assuming that it is truly unsafe, then we aren't going. Simple as that. Get on the phone and call the Chief Pilot and explain your side of the story. Doesn't matter if another FO is there. He isn't assigned to the crew/flight, so he isn't going without you unless the company assigns a new crewmember and redispatches the flight.

2. You’re holding at 5000' in IMC and the GPWS gives you an aural warning "terrain" what are you going to do if the CA says "Oh I get that all the time around here, just disregard"??
Hopefully before you ever reach that situation, you're aware of what's around you. There are times when I've had "spurious" GPWS warnings. If I know for certain that they are wrong- then no biggie... we keep flying. (For example if I get a warning at 15,000 over Virginia). If you don't know for certain, then you execute the escape maneuver.
