I'm not sure why it doesn't show up on my record. Since my failure occurred while on a 135 trip with pax I had to deal with the Feds for a while afterwards, but they assured me that nothing would show up on my record and it hasn't. They advised me that when I fill out insurance forms there is no need to check "yes" on the accident/incident question. Beats me though. It sure felt like an incident when it was happening.
