"I woke up early and had a coffee with two creams and one sugar. The limo showed up late and the driver ran a red light. He felt it was yellow. We got the paper work and hurried to the aircraft. Preflight was normal and we blocked out on time, but I felt very rushed. After the F/O's outstanding takeoff I realized that he had forgot (insert trivial item here). I strongly feel that we should've caught that much earlier. The F/O seemed distracted and his attitude didn't appear to reflect the seriousness of this situation. In the interest of safety FedEx needs to add a three page checklist to ensure this never happens again. At the end of the flight during debrief, I realized I had lost my lucky pen, which I feel contributed to this incident."
As an aside, do you wonder what some of the reports that they don't publish must sound like?
